Lighthouse for Children – Daily Health Questionnaire

This questionnaire must be completed by all individuals for each visit made to the facility. A separate questionnaire must be completed for each individual age 18 and older.

If the terms in which you have acknowledged in this questionnaire are broken, the LFC has the right to ask any one person to leave the facility
